Osteoporosis: screening, evaluation, and monitoring.

چکیده
Michael W. Schaefer, MD, and Geetha Gopalakrishnan, MD osteoporosis is the most common skeletal disorder. It is characterized by a decrease in bone mineral density resulting in fractures. According to the National Osteoporosis Foundation (NOF), 10 million Americans are affected by osteoporosis and an additional 34 million are at risk for osteoporosis. Although osteoporosis is more common among women, men account for approximately 20% of all cases. It is estimated that 50% of women and 25% of men over the age of 50 years will have an osteoporosis related fracture in their lifetime. Osteoporotic fractures typically involve the spine, hip and forearm and result from minimal or no trauma. The morbidity, mortality, and cost associated with these fractures are significant. Mortality rates as high as 25% are reported one year after a hip fracture. If patients survive, 25% will require longterm nursing home care and only 40% will achieve their pre-fracture level of independence. The estimated cost of these fractures to our health care system is more than $17 billion per year and this cost is anticipated to triple by 2040. Therefore, prevention of osteoporosis, particularly fractures, is a public health priority.1

OBJECTIVE (1) To identify patients at risk for osteoporosis through community pharmacy-based bone mineral density (BMD) screening, to refer at-risk patients to primary care and/or specialty practice physicians, and to follow-up with at-risk patients; (2) to treat and manage osteopenic and osteoporotic patients referred to the pharmacy for medication therapy management services; and (3) to test ...
متن کاملA Comparative Study on Willingness to Pay for Breast Cancer and Osteoporosis Screening in Kerman, Southeastern Iran
BACKGROUND One of the economic evaluation techniques involves calculation of willingness to pay (WTP) for a service to find out the value of that service from the clients' perspective. This study estimated WTP for both breast cancer and osteoporosis screening and comparatively examined the contributing factors. In fact, the comparisons served to provide an exact analysis of individual attitudes...
متن کاملMandibular Trabecular Bone Analysis Using Local Binary Pattern for Osteoporosis Diagnosis
Background: Osteoporosis is a systemic skeletal disease characterized by low bone mineral density (BMD) and micro-architectural deterioration of bone tissue, leading to bone fragility and increased fracture risk. Since Panoramic image is a feasible and relatively routine imaging technique in dentistry; it could provide an opportunistic chance for screening osteoporosis. In this regard, numerous...
متن کاملUse of a clinical decision support system to increase osteoporosis screening
BACKGROUND In 2002, the US Preventive Services Task Force recommended routine osteoporosis screening for women aged 65 years or older. However, studies have indicated that osteoporosis remains underdiagnosed, and various methods such as the use of health information technology have been tried to increase screening rates. We investigated whether we could boost the low rates of bone mineral densi...
متن کاملDual-energy x-ray absorptiometry: overused, neglected, or just misunderstood?
Assessment of bone mineral density (BMD) by dual-energy x-ray absorptiometry (DXA) plays a vital role in the diagnosis of osteoporosis and in monitoring a patient's response to drug therapy. This commentary will discuss controversies surrounding the use of DXA for screening and monitoring of BMD in women.
